The following forms will be presented to sign at check-in:
Vaccination Waiver
I, the undersigned, certify that the cat(s) listed on this form has/have received the required vaccinations as outlined in Whisker Ridge Retreat's vaccination policy. I also understand and agree to the following:

My cat(s) is/are in good health and free of any contagious diseases, parasites, or conditions that may harm other feline guests.

I acknowledge that Whisker Ridge Retreat is not liable for any illness or injury that may arise during my cat's stay, including those due to pre-existing conditions or exposure to other cats.

If my cat(s) exhibits symptoms of illness or injury, Whisker Ridge Retreat will contact my emergency contact or veterinarian. If neither is available, I authorize the facility to seek veterinary care at their discretion.

I accept financial responsibility for any veterinary care provided to my cat(s) during their stay.

I certify that my cat(s) is/are in good health, free from contagious diseases, and up-to-date on all required vaccinations (FVRCP and Rabies).

I have disclosed any chronic medical conditions or special care needs to Whisker Ridge Retreat.

Personal Belongings:

Whisker Ridge Retreat is not responsible for loss or damage to personal items (e.g., toys, blankets). All items must be clearly labeled.

Behavioral Agreement:

I have disclosed all known behavioral issues (e.g., aggression, anxiety).

I understand that Whisker Ridge Retreat reserves the right to handle my cat(s) as needed to maintain safety.

Emergency Medical Authorization:

In the event of illness or injury, I authorize Whisker Ridge Retreat to seek veterinary care for my cat(s) at my expense.

Payment Policy:

A 50% deposit is required at the time of reservation. The remaining balance is due at check-in.

Additional fees, such as early/late pick-up charges, are due at check-out.

Accepted payment methods: Card, Venmo, PayPal.

Early Check-In/Late Pick-Ups:

I understand that Check-In Time is between 9:00 and 11:00 AM and Check-Out Time: Between 12:00 and 5:00 PM

Early Check-In will incur an additional fee of $15.

Late Pick-Up will incur an additional fee of $15
